Letters regarding treatment will contain objective information regarding history of engagement including compliance with treatment recommendation, start of treatment date, date of most recent session. Please indicate if you would like your diagnosis to be included. This letter is provided only to client for client's use as they desire. No release of information is needed as there will be no communication with third parties. 
 
At this time, I only have access to Grow and Headway's records. If you worked with me on Thriveworks you will need to provide date of initiating treatment. 
 
Changes to this letter requiring more intensive use of provider's time will result in further charges. Letter will be provided to you within one business week of the attached document being filled out and returned to sarah@luminouscounselingjourney.com via email.
