Paperwork and forms will only be filled out for client when appropriate. If it is determined to not be ethical or appropriate to complete documentaiton a full refund will be given. Need for document should have been discussed previously in session. Documentation can only be ethically provided for clients who are currently active in treatment. 
 
Changes to documentation requiring more intensive use of provider's time will result in further charges.
 
Letter will be provided to you within one business week of the below information being sent to sarah@luminouscounselingjourney.com:
1. Document emailed to you after purchase
2. Documentation or forms needing to be filled out by provider
3. Any supplementary information or context
